The Palliative Care programme is available across all practices.
The registration dates can be viewed by any practice.

Forms

The Palliative Care programme has three forms:

  1. New Registration – onto the Palliative Care programme
  2. Claim – for each consult
  3. Exit/renew

Programme notes

  • If the programme expiry date is close, use the Exit/Renew Patient form to extend the programme expiry date for another 6 months.
  • If the patient no longer needs the programme, use the Exit/Renew Patient form to remove the patient from the programme.
  • There is a limit of one claim per patient per claim type per day. Multiple claims for the same claim type will be submitted as ‘Requires review’. These claims are reviewed at Health HB and you may receive a call to check if the claim is valid.
  • Patients are not required to be enrolled with the practice.

The new forms use the 2018 CVD Risk Guidelines and the new calculator.

The Diabetes form has a separate link (previously the Diabetes & CVD form were the same).

There are two links to the CVD form:

  • funded
  • non-funded

Updated Cardiovascular Risk Calculator

The Cardiovascular Disease (CVD) Risk Assessment calculator in the Halcyon forms for cardiovascular risk and diabetes uses the New Zealand (NZ) Primary Prevention equations.

The risk categories and thresholds for pharmacological management of cardiovascular risk are updated with moving to use the NZ Primary Prevention equations.

The CPO programme for Acute Care, Gynae, Residential Care, and Contraception pathways has two forms:

  • Referral
  • Claim / outcome

CPO Hospital Discharge has it’s own form as it doesn’t require a referral. Click here to see more about CPO Hospital Discharge.

Referrals

Create a new referral and choose the pathway.

A link to the claim form for each pathway will appear on the Current Programmes tab.

Claims

The claim form has been streamlined

  • A link to the claim form for the open pathway will show on the Current Programmes tab when a referral is open
  • If there are multiple pathways open, there will be a link for each pathway (see image below)
  • The CPO number is prepopulated when you choose the pathway to make a claim for (see image below)
  • Complete a new claim form for each item or service provided
  • A claim can’t be made without an open referral

Outcomes

A summary of the referral and claim information is shown on the Current Programmes page.

  • Choose an outcome option on the claim/outcome form
  • A final diagnosis box will appear
  • Choose the outcome diagnosis (this will prepopulate if there is only one possible final diagnosis)
  • Add clinical notes and update the management section.
