Clinical Information from Clinical Portal
Patients Medication History using Manage My Health Primary Care Record
UPDATE 23/12/22 – Information for Te Mata Peak Practice, Maraenui Medical, Hauroa Heretaunga, and Taradale Medical is now available in the Primary Care Record.
1. Log into HBDHB Portal/Citrix -> Clinical Portal -> Demographic Search -> Summary tab
2. Select Primary Care Record from the left-hand menu of the summary tab -> launches new window
3. Select Medications Tab -> grouped by date order
4. Select Diagnosis = patients conditions – > grouped by date of entry
Blood test Results (Go back to Clinical Portal – not in Manage My Health)
1. Select Laboratory from the left-hand side of the summary menu
2. Select Biochemistry for renal function OR haematology for full blood count/blood count
3. eGFR can be found in either Renal Function or Biochemistry
