Enrolling with General Practice

This webpage is to support practices and patients with practical information and documents to assist with the enrolment to a general practice and to comply with the PHO enrolment rules required by Te Whatu Ora.

When you enrol with one of our general practices you are automatically enrolled with Health Hawke’s Bay, Te Oranga o Te Matau-a Maui.  This means that we will subsidise your visits to that general practice.

How do you enrol?

Are you eligible to enrol?

Any person who is eligible for publicly funded health services can choose to enrol. For information regarding who is eligible refer to the guide Enrolment guide for patients

**Recent changes

The Health Hawke’s Bay enrolment forms have been recently updated in August 2024. Key changes include amending sex at birth and gender information, adding the collection of iwi and hapu information, removing the CSC number, updating the eligibility criteria (a) to include people from Cook Islands, Niue or Tokelau) and removing the wording “OR in the control of the Chief Executive of the Ministry of Social Development’ from (g).
